Intake/Measuring Success

INTAKE

  • Each applicant will be screened by Mary’s Help’s Social Worker/Administrator, Treatment Team and the applicant’s authorized representative.
  • An appropriate level of individualized care will be jointly developed.
  • A written Needs and Service Plan will be prepared incorporating designated services.
  • Services may include:  Educational, Mental Health and Social Work Services. (This document along with an admission agreement will be signed by Mary’s Help Social Worker/Administrator, Treatment Team members and the placement agency representative prior to admission to the program.)

MEASURING SUCCESS

Completion of measurable Individualized Treatment Goals based on Child and Family Team Meetings (CFT) and Needs and Service Plan.  The philosophy of Mary’s Help THPP/THP + FC is to design and develop the right combination of social work services, case management, and psychological support to develop successful and effective individualized treatment programs.  These programs are designed specifically for young adults with an extensive childhood history of underlying emotional diagnoses as the result of abuse, neglect, medical, psychological, and learning disorders.
